What is Guaranteed Standard Issue (GSI) disability insurance?

GSI is an individual disability insurance program available exclusively to medical residents and fellows at participating hospitals. It provides guaranteed approval with no medical exams, lab work, or health questions, along with True Own-Occupation coverage tailored to your medical specialty.

How does GSI differ from my hospital’s group long-term disability plan?

Your hospital’s group LTD plan is a valuable benefit — but it is not the same as individual disability coverage. Key differences:

•       Group LTD typically uses a broader disability definition (“any occupation”) rather than specialty-specific coverage

•       Group LTD ends when you leave your hospital — GSI is portable and belongs to you

•       Group LTD benefit amounts are set by the plan — GSI is customizable

•       Group LTD cannot be increased without new underwriting — GSI has flexible increase options

When is the deadline to apply for GSI?

You must apply while you are still in residency or fellowship at a participating program. GSI eligibility expires within 90 days after graduation. Most physicians benefit from applying in PGY-1 or PGY-2 to lock in the lowest rates and protect as much of their training window as possible.

What happens to my GSI policy after I graduate?

Your policy stays with you. It is an individual policy — not tied to your hospital or employer. Your discounted training-year premium rate remains in effect, and you can increase your coverage using the FIO or BPR rider as your attending income grows, without going through medical underwriting.

What if my health changes before I apply?

That’s exactly the risk GSI protects against. Since no medical underwriting is required, any pre-existing conditions you have at the time of application do not affect your eligibility or premium. However, if a new health issue arises before you apply, it could affect your ability to obtain traditional individual coverage in the future. Applying during training protects your insurability now.

Can I apply if I already have a group disability plan at my hospital?

Yes. Having group LTD coverage at your hospital does not disqualify you from GSI. Many residents have both — the group plan provides a baseline, and GSI provides portable, specialty-specific coverage that stays with them throughout their career.

What happens if I apply for other disability insurance first?

This is important: if you apply for individually underwritten disability insurance through another carrier first and receive a modified offer or decline, you may permanently lose your eligibility for Guardian GSI. Talk to an advisor before submitting any disability insurance application.
